Nanotechnology has emerged as a transformative paradigm in precision medicine, enabling the rational design of intelligent nanocarriers that overcome biological barriers and deliver therapeutic payloads with unprecedented specificity. This comprehensive chapter examines the multidisciplinary landscape of nanobiotechnology, synthesizing recent advances in nanomaterial science, protein corona engineering, stimuli-responsive systems, cellular trafficking mechanisms, and tumor microenvironment remodeling. We evaluate quantum confinement effects in inorganic nanoparticles, the protein corona paradigm as a biological identity determinant, and hierarchical multi-stimulus responsiveness achieving 500–1000-fold therapeutic selectivity. Critical mechanisms of cellular uptake, endosomal escape (40–50% efficiency in optimized systems), and immune activation are detailed with quantitative metrics. Clinical translation is substantiated through FDA-approved nanomedicines spanning 30 years of development, with emphasis on mRNA-lipid nanoparticles representing the first successful clinical demonstration of rational nanocarrier design principles.
